HIV Care Management

HIV Care Management

The integration of various disciplines allows GLFHC to provide a full spectrum of support to over 330 HIV patients. Our HIV team consists of Primary Care Providers, HIV Specialists – AAHIVM-certified Family Physician and Physician’s Assistant, HIV Nurse Care Coordinator, Medical Case Managers, Peer Navigator, Clinical Pharmacist, Nutritionist, and Behavioral Health professionals.

School-Based Health Centers

GLFHC operates two School-Based Health Centers at Lawrence High School and Greater Lawrence Technical School. These clinics, in partnership with the school system’s nursing staff, provide accessible clinical care, social services, health education, nutrition, and counseling to the students enrolled there. GLFHC’s patients can be seen during school hours for physicals, acute care, and immunizations, keeping them healthy while reducing time out of school.
