Primary Healthcare
Greater Lawrence Family Health Center offers primary healthcare for children and adults, including prenatal care, obstetrics, pediatrics, elderly care, gynecology, internal medicine, disease prevention and health education, nutrition counseling, and group health visits.
GLFHC providers are Board-certified Family Medicine Physicians, Nurse Practitioners, and Physician Assistants. Because of their extensive training, family physicians are the only specialists qualified to treat most ailments and provide comprehensive health care for people of all ages– from newborns to seniors. GLFHC providers deliver a range of acute, chronic, and preventive medical care services while providing patients with a patient-centered medical home.
In addition to diagnosing and treating illness, they also provide preventive care, including routine checkups, health-risk assessments, immunization and screening tests, and personalized counseling on maintaining a healthy lifestyle. Family physicians also manage chronic illness, often coordinating care provided by other subspecialists. From heart disease, stroke and hypertension, to diabetes, cancer, and asthma, family physicians provide ongoing, personal care for patients.

Specialty Care and Referrals
Greater Lawrence Family Health Center can provide many specialty services on-site. For access to external specialists, GLFHC has referral and coordination agreements in place with specialty providers in the Merrimack Valley.
Laboratory Services
Greater Lawrence Family Health Center offers laboratory services within its clinic locations, so results are available quickly and treatment can be provided right away.
Culturally Competent Care
Greater Lawrence Family Health Center strives to provide culturally competent care throughout all of its programs and services. All perinatal, pediatric, and adult primary care services are linguistically and culturally sensitive to patients’ needs. 62% of staff members are bilingual/bicultural, and all of our providers speak Spanish. All GLFHC staff members receive cultural competency training. Live translation services are available for 17 other languages, and GLFHC makes every accommodation for patients needing assistance in a language other than English.
The Patient-Centered Medical Home (PCMH) model promotes partnerships between patients and their primary care clinicians.
PCMH creates a medical team who will provide for all of a patient’s health care needs and coordinate treatments across the health care system. Medical home clinicians demonstrate the benchmarks of patient-centered care by implementing open scheduling, extended clinical hours, and an effective use of current and expanded health information systems.
In the medical home, care is evidence-based, accessible, well-coordinated and patient focused. Most importantly, the patient’s voice is heard, and patient satisfaction and quality of care are of the utmost importance. Greater Lawrence Family Health Center has achieved Level 3 PCMH Recognition from the National Committee for Quality Accreditation.
Your PCMH team will include your provider, nurse, and medical assistant. Depending on your unique needs, your team may also include a clinical pharmacist, nutritionist, specialist, and behavioral health expert. Your team will work with external providers and hospitals to ensure coordination throughout the continuum of care. You will always be informed about your care options, and GLFHC patients have access to the Patient Portal, an online self-service system, where you can connect with your provider.
